I agree. If you're trying to bench your max with a false grip, this kinda shit happens. I tend to use the false grip when I bench low weight with high reps. It's a bad habbit, but I don't do it when I'm using a heavy weight.

There's a reason the false grip is also called a suicide grip. It looks like he was never fully set in the first place - he kept shifting his grip around prior (racking, re-racking). Definitely not the spotter's fault if you decide to drop it on your face.
