The grenande has a "spoon" on it that holds back a spring-loaded firing pin. When the the pin is pulled you must hold the spoon in place with your hand. When the grenade is thrown, the spoon flies off, releasing the firing pin which ignites the (3-5 sec) time fuse that detonates the main explosive.
People in the movies release the spoon which goes "ping" and hold the grenade for a second or two before throwing but that's pretty much hollywood bullshit. When he released the spoon that fucking firing pin hit the primer and made a loud noise which scared him and he fumbled the grenade.
