Not a bus. Definitely a subway or a light rail train of some kind. And during commute, I can believe that there are places in the US where such trains would be packed like sardines in a can.
But fighting in a situation like that is just retarded. Other people around them should have worked harder to stop it. I would have just put my arm around the guy's throat and yelled at him to STFU and GTFO at the next stop.

There are a few cities in the US packed this tight. Depends on the time of day etc. But mass transit is far more popular here than most people realize. Bike riding is having a huge come back also with many cities putting in massive bike routes and trails. Around me there are over two hundred miles of them (Midwest US). Some of it is exercise based and some are riding bike's for pollution and energy savings. The nearest trail to me is less than 4 blocks and goes through several states.

nor do they have those yellow cables on the sides to call your stop, seeing as they stop at every station
nor do they generally have stairs (at least, in montreal busses have stairs, while subways don't)

The light rail trains where I live have step wells like buses, and pull-cords. They also have drivers in the front, and mirrors.
