I've never understood the optimism behind tunnel design. The designers seem to assume that no car will ever break down inside a tunnel. And so, there doesn't need to be any place for cars to stop. Yeah, I know it costs more to make tunnels bigger. But the damage caused by explosions isn't exactly free.

The truck on the left blew a tyre. The truck that caused the crash was doing double the speed of the cars that had slowed for the stopped truck and smashed several other cars. 3 people died, and the truck driver who caused the deaths is now on trial in Melbourne.
